Referrals & Access to Services
Integrative, wellness-based sessions are available through self-referral and private payment.
When appropriate, insurance billing may be utilized for medically necessary services.
For medically necessary services, we will need:

A prescription order for "occupational therapy" from your physician. You may get this yourself, or we can request it from your physician.

Insurance Information: A copy of your insurance card(s) is required prior to your initial session.

Completion of intake forms.
