Referrals & Access to Services

You’re welcome to reach out directly to schedule services, or referrals may come from physicians, families, and community organizations.

When services are determined to be medically necessary, insurance billing may be utilized. Integrative and wellness sessions are also available for self-referral and private payment.

To Begin Services, We Will Need

A prescription order for "occupational therapy" from your physician. You may get this yourself, or we can request it from your physician.

Insurance Information
Copies of your insurance cards are needed prior to your first appointment. If you have both a primary insurance and Medicaid, please provide copies of both — Medicaid requires that the primary insurance be billed first.

Completion of intake forms.
